You pick the services you expect to use, tell us how to bill you, and name the people who receive each type of result.
Have on hand
- Company legal name, DBA, and address
- Billing contact and the email address for invoices
- Workers' compensation carrier, policy number, and the TPA or adjuster contact if you have one
- Your designated employer representative for drug and alcohol testing
- The names, secure email addresses, and secure fax numbers of the people who receive drug and alcohol results, physical results, and injury and work status updates
