Privacy policy

This policy covers this website. It does not cover the care you receive at the clinic; OMP’s Notice of Privacy Practices, which you receive at the clinic, covers your care and your medical information.

What this website collects

Nothing you type into a form is stored on this website. The four forms on this site (become a client, send an employee, request an appointment, contact) are provided by a separate form vendor. OMP uses a form vendor for these forms only under a business associate agreement. When you submit a form you are sending it to that vendor, which delivers it to OMP’s front desk. This website never receives, logs, or stores a submission, and the address bar never carries what you typed.

Analytics. This website uses Google Analytics to count visits and see which pages people use. Analytics records the page you viewed, the button you clicked, and the general location and device type your browser reports. It does not record names, form contents, or anything you type.

Call tracking. The phone number shown on this website may be a tracking number that forwards to the clinic’s main line, so OMP can tell which pages lead to calls. Calls are not recorded.

Cookies. Analytics and call tracking may set cookies to tell one visit from another. This website shows no advertising.

What this website never does

• It never displays, stores, or transmits test results, work status, or any medical record. Results go to the people named on an employer’s service agreement, by the channels the agreement names.
• It never puts personal or medical information in a web address, an analytics event, or a call tracking note.
• It never sells or shares visitor information.

Links to other sites

The address links open Google Maps. The employer portal, when it opens, is a separate system with its own login and its own privacy terms. This policy does not cover those sites.

Changes

OMP may update this policy. The date at the top is the date of the current version.
