Questions we get asked

Photo ID, glasses or contacts, hearing aids, a medication list, a CPAP compliance report from the driver's doctor if the driver uses one, reports from any other periodic testing the certification requires, any specialist letters, and the DOT form if the employer provided one.

Yes. Pre-employment, random, post-accident, reasonable-suspicion, and return-to-duty testing under 49 CFR Part 40, with results to your designated employer representative.

Yes. The lot at 1714 Charlestown New Albany Road has room for a tractor-trailer, off I-65 at Exit 4.

No. A driver can walk in by 4:00pm, Monday to Friday, or you can send an authorization ahead and the front desk schedules the exam.

A certified medical examiner listed on the FMCSA National Registry. The examiner completes the exam and issues the medical certificate card when the driver qualifies.

The driver leaves with the medical certificate card when they qualify, and you get a copy after the visit.

Sign the service agreement online. It sets up billing, your workers' compensation carrier and TPA, your designated employer representative, and who receives each type of result.

Submit the send-an-employee authorization with the employee's name, date of birth, phone, and the services you are authorizing. You get a confirmation and the employee gets a what-to-bring message. Not a client yet? Sign the service agreement first.

For an emergency, call 911. Otherwise send the employee to OMP during clinic hours, Monday to Friday 8:00am to 5:00pm, with your authorization. Work status and restrictions are communicated to you after the visit.

Not always. A DOT physical or a pre-placement physical can walk in by 4:00pm or be scheduled. An extended exam with several tests, such as labs, an audiogram, or spirometry, is by appointment. Send an authorization or call 812.271.4240 and the front desk sets it up. A fit test prefers an appointment and accepts walk-ins.

No longer than about 30 minutes to be seen. A DOT physical or a pre-placement physical usually takes about 30 minutes. An extended exam with labs, an audiogram, or spirometry can take up to an hour and is scheduled in advance. A fit test is usually up to 30 minutes; a vaccine alone is about 15 to 20 minutes. A drug screen or breath alcohol test takes up to 30 minutes, usually less. An injury visit can take up to an hour, depending on the injury.

Dr. Dennis McClain, the medical director, is in the clinic two days a week. On the other days a nurse practitioner trained by Dr. McClain sees patients. The clinic manager, Barbara Elliott, is a nurse practitioner with 30 years in occupational medicine.

The employee leaves the visit with a return-to-work form showing any restrictions, to bring back to you. OMP also sends a visit summary to the employer contacts named on your account, by email or fax.

No. OMP does not have imaging in the clinic. When an employee needs an x-ray or other imaging, OMP refers them to an outside imaging center.

Drug and alcohol results come to you through an eScreen account, which OMP sets up when you become a client; OMP does not send them or call with them. If your company has a national eScreen account, results go through that account, and OMP does not see them. Physical exam results and work status go to the recipients you named for that result type.

Yes. Dr. McClain or one of our nurse practitioners will talk with the employer, adjuster, or case manager when a case needs it. Call 812.271.4240 and the front desk arranges it.

Yes. Onsite vaccine and screening days are scheduled through the front desk. A participant minimum and a cancellation fee apply under the service agreement.

OMP bills your company for the services you order, such as drug screens, breath alcohol tests, and physicals, unless a third-party administrator pays for them. Invoices are emailed weekly and are due in 30 days. You can pay by ACH (preferred), check, or credit card, which carries a 3 percent surcharge. Workers' comp visits are billed differently; see "How are workers' comp visits billed?"

Your company can pay the invoice, or OMP can bill your workers' comp carrier directly. To bill the carrier, OMP needs the carrier's name and the claim number. If the carrier denies the claim, OMP bills your company.

OMP is at 1714 Charlestown New Albany Road in Jeffersonville, Indiana, across the Ohio River from downtown Louisville, off I-65 at Exit 4. The lot has room for a tractor-trailer.
